In part: "Witnesseth: That for consideration hereafter mentioned, the said Dr. Henry Glasspoole promises and agrees to perform the duties of a Medical Officer, agreeably to Army Regulations, at Madison, Wisconsin or elsewhere, and to furnish and keep in good order, and accessible at all times, complete sets of amputating, trephining, and pocket instruments, and the said Surgeon T. M. Getty U.S.A. Med. Dir. Dept. North West promises and agrees, on behalf of the United States, to pay... the sum of one hundred dollars for each and every month he shall continue to perform the services above stated..." If the doctor could not provide instruments, the Army would supply them and deduct the cost from his pay. The doctor would also receive a slightly higher salary if he performed in the field, as well as a transportation allowance. Signed and witnessed by all appropriate parties. Soiled on the outer docketing surfaces; otherwise clean and intact. Extremely Rare - the first such form we've seen.
