A rare war-date partly-printed Confederate surgeon's military exemption document, 1p. oblong 8vo., issued to forty-three year-old Henrico, County, Va., farmer Thomas L. Pleasants at Richmond, Va., July 6, 1864, "on account of excessive hemorrhoids. We furthermore declare this disability to be permanent." Signed by a board of three surgeons consisting of C. D. Rinsley[?], A. P. Hall and John Mayo and approved, on the verso, by Capt. John Archer Coke, formerly commander of Coke's Williamsburg Artillery, as enrolling officer for Virginia's 3rd Congressional District. Printed on lined necessity paper.
